VB6 PDW deployment causes Windows Explorer GPF

My development environment is NT/2000, VB6 service pack 4.
I use the Package and Deployment wizard.

On two NT 4.0 clients, installation of my app caused Windows explorer to GPF to Dr. Watson after installation. Couldn't even re-boot. Had to rebuild their systems.

Any ideas on WHAT I'm deploying that could be causing this? Here's the contents of the
